Please submit this application to receive permission to operate a temporary food and/or drink facility within Sotorik city limits during a special event. Supplementary materials (required unless specified otherwise): (a) Photographs of all facilities and equipment to be used on-site for food/drink preparation and service (b) A list of all food/drink items to be sold and the ingredients of all of the non-prepackaged items (c) A check payable to Sotorik City Hall for $50 OR, if the application is being submitted fewer than 10 days before the event, for $100 (d) A certificate of inspection from the Sotorik Department of Health (only required for vendors intending to use gas-powered appliances in their facility)
